provide a quality service appropriate to you and your dog's needs
Providing this information is optional however, if you choose not to enter the necessary information, we may be unable to provide you with our services. Your dog may be at risk of harm if you do not provide their relevant medical history before engaging our services or keep us up to date with any new medical issues, should they arise.
We keep your information safe by storing it on a secure cloud-based server. We keep your information for as long as is necessary to provide you and your dog with a quality service. Your information will be deleted upon your specific request in writing. A copy of session notes can be provided upon request.
